PHAL: improved boot failure reason code based logging

Added support to all newly added ipl failures.
 - IPL_ERR_HWP reports hwp procedure failures
 - IPL_ERR_SBE_CHIPOP reports during continue mpipl chip-op
 - IPL_ERR_SBE_BOOT reports sbe_start step.
 - All remaining failures log generic boot failure now

Tested: Manually verified.

To Build

To build this package, do the following steps:

    1. ./bootstrap.sh
    2. ./configure ${CONFIGURE_FLAGS}
    3. make

To build with phal feature:
    1. ./bootstrap.sh
    2. ./configure ${CONFIGURE_FLAGS} --enable-phal --enable-openfsi
    3. make

To clean the repository run `./bootstrap.sh clean`.
